As a provider of aged care, disability and community services, Omnicare collects personal information from our clients and other third parties. The collection and proper management of personal information is taken very seriously by all staff at Omnicare and we aim to ensure that your information is only used with your knowledge, consent and for your benefit.

We collect a variety of personal information from our clients, staff, volunteers, contractors, and stakeholders in order to provide and improve our service delivery. In particular it is important for us to have full and accurate client information so that we can assess their specific needs and provide them with appropriate services and care.  Without access to a client’s personal information, it would be very difficult for Omnicare to provide them with assistance.  The type of information we collect will depend on which of our services are being used. Examples of personal information which may be collect include:

  • Contact details (name, address, phone number and email).

  • Demographic information (age, gender, disability and medical information).

  • Marital status.

  • Family details, including personal information about a client’s next of kin.

  • Health records, care plans and assessments.

  • Government identification (driver’s license, centrelink reference number, medicare card).

  • Financial details (for billing purposes, where applicable).

 

We only collect personal information that is necessary for the purpose of delivering our services and will do so in a lawful and transparent way.

Potential clients can make anonymous enquiries to Omnicare in relation to our services. Without having access to specific personal information, Omnicare would only be able to provide general information pertaining to costs and services.
